Click APPLY NOW! to access the Online Application Form Select your desired Certification Program Fill out your personal information Select your payment option and method of payment Enter your name and application date in designated areas Press the Submit Button to forward your online application form to FTU. Once your online application form is received, we will send an Enrollment Agreement to your email address. You may sign and forward your Enrollment Agreement along with your tuition payment to begin your Certification Program.
